MEDICATION POLICY All medication MUST be brought to the school office by a responsible adult. Medication must be brought to school in the original, pharmacy-labeled container. The container shall display the student’s name, prescription number, medication name and dosage, directions, date, licensed transcriber’s name, pharmacy name, address and phone number. Over-the-counter drugs require an order from the licensed prescriber. The medication must be in an unopened container with the manufacturer’s original label and ingredients listed. The student’s name must be affixed to the container. The physician’s order and parental consent form MUST be used for ALL medications. A copy of the “Authorization for Medication During School Hours” form can be found on the school website or you may pick one up in the main office.
